We are committed to providing you with the best possible care. If you have insurance coverage for the services provided, we will help you receive your maximum allowable benefits. In order to achieve these goals, we need your assistance, and your understanding of our payment policy.
You must realize that your insurance is a contract between you, your employer and the insurance company. Not all services are a covered benefit in all contracts. Some insurance companies arbitrarily select certain services they will not cover.
The following payment plans have been established for our patients.
- Payment in full at the time services are rendered.
- Use of Visa or Mastercard
- Payment plans which are available and will be thoroughly discussed with you
- All materials must be paid for in full before they are released from the office
Please bring an insurance form with you to the office on your first visit, with your portion completed accurately. Please remember that no insurance company attempts to cover all costs. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, coinsurance, or any other balance not paid for by your insurance company.
Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment.
